Well, when you go into your game, you don't actually physically see an option to select the skin you downloaded. They will automatically be used on your sims and there's not a huge difference. They still look basically the same but the stomach area of male sims will look more toned and so forth....

Oh, you should be ok to delete whatever the other files are. They are most probably just images of the download, I'm assuming As long as you've put the .package file and resource file where they're meant to go, the skins should work.
